We provide specialized support for individuals eligible for Montana’s Adult Medicaid Mental Health Services. Residents accepted into this category typically face significant mental health challenges that require structured support and professional care. This includes:

  • Adults who qualify for the Medicaid Severely Disabling Mental Illness (SDMI) Waiver

  • Adults eligible for Adult Foster Care Home Placement

Some Qualifying Circumstances:

  • Individuals who require assistance with daily living activities

  • Vulnerable adults in need of a structured and supportive environment

  • Those who are transitioning from a state hospital or other institutional care to community-based living

  • People who benefit from life skills training, vocational support, and therapeutic services

  • Youth who have aged out of child foster care and still need assistance

  • Adults who struggle with diagnosed severe mental illness

  • Those facing homelessness

How to Apply for Residency

At Empowered Living Solutions, we are committed to providing a smooth and supportive process for those interested in applying for residency at our homes. Here’s how you can begin the journey:

  • Get Your Support Needs Assessed:

If you or a loved one may qualify for Medicaid services in Montana, the first step is to have your support needs assessed. You can start the process by contacting Mountain-Pacific Quality Health at (406) 443-4020, which handles Montana Medicaid Utilization Reviews. You will also need to reach out to AWARE at (406)563-8117 for SDMI Case Management services. They will help and assess your eligibility for Medicaid waiver programs, such as the Severe Disabling Mental Illness (SDMI) Waiver or other qualifying services.   • For Residents Being Discharged from Montana State Hospital:

If you are being discharged from the Montana State Hospital, you may also qualify for residency at one of our homes. The same process applies: contact AWARE and Mountain-Pacific Quality Health to schedule your assessments.

  • Waiver Recipients and Waiting List Candidates:

Empowered Living Solutions accepts residents who have already received waiver benefits as well as those who are currently on the waiting list for waiver services.
